Financial Responsibility Agreement
Welcome to Summit Oral & Facial Centre. The Summit Centre’s mission is to provide a full range of procedures including specialized aesthetic, diagnostic and surgical services in oral and maxillofacial care to patients from across Prince Edward Island. Summit Centre’s surgical and administrative team is dedicated to ongoing excellence in caring for our patients before, during and after their procedures. Our modern facilities, friendly staff and experienced surgical team ensure a positive experience for every patient and their family.
Our clinic policy for patients scheduled for a procedure is that fees are due and payable on the day of service. Consultation appointments are payable on the day of the consult. Payments can be made using cash, interac, credit card, and etransfer ([email protected]). Regretfully, we are no longer able to accept personal cheques.
We will be happy to provide you with an estimate of cost, as well as a pre-authorization, so that you may determine coverage based on your insurance policy. If you haven’t received a pre-determination from your insurance provider, we recommend that you follow up with them prior to your appointment. The insurer DOES NOT provide us with a copy. We ask that you provide a copy of the pre-determination to the office on or before the day of your appointment. We need this in order to determine the amount of co-pay and without it we will have to charge you the full amount of the procedure. We do ask that payment for services rendered be paid on the day they are provided. Alternate arrangements should be discussed, in advance, with a member of our business staff.
It is important to remember that you are fully responsible for all fees charged by this office regardless of your level of insurance. Insurance estimates are estimates, and the actual amount covered may vary from the estimate. Patients are responsible to pay any amounts not covered by the insurer.
Cancellation Policy
To cancel or change an appointment, please call (902) 892-7600 during regular office hours. While telephone messages may be left after hours on our answering machine, patients must contact us during office hours about cancellations. There will be a fee for a missed appointment with fewer than 48 hours notice. The fee charged will be 50% of the cost of the procedure, as medications and staff for the procedure have been prepared in advance. The procedure will not be rescheduled until the fee is paid.

Acknowledgement
I have read and understood the above Financial Responsibility Agreement. I understand that I am responsible for all fees associated with the services I receive. I also understand that a service charge of 10% per month will be applied on all outstanding balances. I consent to the collection, use, and disclosure of my personal information by Summit Centre, as permitted by law, for the purposes described in this document.
